Year 9 Transition Review Meeting

Children and young people with an Education, Health and Care (EHC) Plan have an Annual Review every year throughout their time at school or college. The Annual Review checks that the support they receive is still appropriate and effective.

But Transition Review Meeting in Year 9 marks the start of planning for your transition to adulthood.

This means thinking about what you will want to do when you leave school, including:

  • Your next steps in education
  • Training and employment opportunities
  • Becoming more independent, including where you might want to live
  • Being part of your community (friendships, sport and leisure)
  • Qualifications and skills you will need to achieve their goals
  • Health pathway, including a review of factors relating to your health

All young people must remain in education, employment or training until age 18.

Review meetings will continue to be held as the young person moves through their school and college years. EHC plan reviews must be person-centred, which means that your aspirations for the future will be key considerations for every decision made.

Your teacher or SENCO should help you to prepare to share your hopes and goals for the future at the review meeting.

Who goes to the Transition Review Meeting?

There are people who must be included in the transition review meeting.

  • Your parent or carer.
  • The young person (you may not wish to attend the meeting in person, but your views must be considered).
  • The special educational needs coordinator (SENCO) or teacher who will chair the meeting.

There are a range of other professionals who can attend a Transition Review. Who should be invited to the Transition Review will depend on your individual needs and can include:

  • SEND Case Officer/SEND post-16 Case Officer
  • relevant health practitioners
  • relevant education practitioners
  • Independent Parent Supporter
  • the Transition Team from Surrey County Council
  • social worker
  • representatives of post-16 providers that you may be interested in (particularly if this transition review is taking place in Year 11)

How is the meeting recorded?

Your school will prepare the paperwork for the meeting. The necessary documents can be found below or from the downloads section of this page.

Surrey EHC plan statutory review form is used at every EHC plan review meeting, at all ages. It charts your progress, how well your EHC plan has been working for you, and will help to identify any changes needed to the support you are given.

The Moving Forward document needs to be used from Year 9 onwards, to keep a record of the plan for helping you to prepare for adulthood.

I have SEND but don't have an EHC plan. What transition support can I get?

A young person identified as having special educational needs has the right to support. This includes young people whose needs can be fully met without having an EHC plan.

Young People with SEN will have an Individual Support Plan (ISP) at school to support your learning, and this must be regularly monitored to ensure the support is still right for you.

From the age of 14, you should be encouraged to think about your aspirations for the future. This will become a focus point each time your Individual Support Plan is reviewed.

There are other ways you can prepare for the transition to adult life including career events, college open days and work experience.
