Services supporting young people post 16

As a child becomes a young adult, the services they work with may need to change to meet their needs. From fourteen years old plans should be made in the EHCP review meetings, including what support the young person will continue to need when they leave school.

The aim of this page is to help you find the right team to get the support available to young people as they become adults. If you are unsure about who can help you, please complete the Local Offer 'contact us' form and a member of the team will help you.

The following list of services are in alphabetical order.

Adult Social Care

Rights and Participation Team

ATLAS is a network of children and young people who have accessed any SEND service within Surrey. The aim of The Rights and Participation Team is to help change, shape and develop the services using service user experience in a fun and creative way. There are two participation groups that meet in Woking and Epsom to discuss changes within the service and to have young people's voices heard. The ATLAS motto is "No decision about us, without us."

If you would like more information or would like to get involved with ATLAS then please contact the team on rap.team@surreycc.gov.uk or by calling 01483 519571.

SEND Team

SEND Case Workers are allocated to a child when they are being assessed for an Education Health and Care (EHC) plan. They remain allocated to the child once their plan is in action. SEND Case Workers provide advice and support to both school and parents and can attend review meetings when more guidance is needed.
