Learners' Request for Support

Learners' request for support has been created to help learners across Surrey get support when they need it.

It is designed to make sure we get a complete view of a child or young person's needs. Helping to focus on exactly what support the individual needs. The forms below should be used by any professional working with a child or young person, when they have a concern about their learning and/or development.

You can submit these documents, along with any other supporting documents, by using the Request for support online form.

Please be aware that the online form will time out after 30 minutes, so it's best to have all information and documents to hand before you begin.

What to include:

You must include the following key information:

  • The child or young person's name, date of birth, address, contact phone number and unique pupil number
  • Parent or carer information, including name, email and phone, and address (if different to the child)
  • Schools attended in the last 2 years, with start dates (maximum 5 schools)

You can also upload supporting documents:

  • SEND Support Arrangements
  • Assessment Reports (no later than the last 2 years)
    • Education Psychologist
    • Occupational Therapist
    • Speech and Language Therapist
    • Medical
    • Health Care
    • CAMHS
    • Social Work
  • Provision map with costings
  • Academic progress over the last 3 years

There is a total limit of 30MB for all documents uploaded (and 20MB maximum for a single document).

Please note, that the online portal cannot accept Zip (.ZIP) files. If you have additional documents that you are not able to upload, you can email them to learnerssupport.triage@surreycc.gov.uk.

The online form will time out after 30 minutes, so have all information and documents to hand.

What happens next

Our team will review your request for support and ensure that appropriate next steps are taken, based on the child or young person's individual needs.
